Sample Report: Pacemaker Generator Replacement

DIAGNOSIS: Tachycardia/bradycardia syndrome.

PROCEDURE: Pacemaker generator replacement.

DETAILS OF PROCEDURE: Following informed consent, the patient was brought to the catheterization lab in a postabsorptive state. Anesthesia was performed with intravenous fentanyl and Versed. Local anesthetic was infiltrated subcutaneously. The patient received prophylactic intravenous antibiotics. Local anesthetic was infiltrated subcutaneously. A #15 scalpel was used to incise the skin over the left chest. Blunt dissection requiring hemostasis was used to expose the chronically implanted generator. The chronic generator was explanted and the leads detached. Ventricular lead showed threshold 0.7 V, resistance 600 ohms, R wave 18 mV. This is a model 1216. The atrial lead was capped due to chronic atrial fibrillation. The leads were attached to a St. Jude model 5172. Pocket was lavaged with antibiotic solution. Subcutaneous layer was closed with 3-0 Vicryl. Cutaneous layer was closed with 4-0 Vicryl. There were no apparent complications.
